My name is Luca, I’m 40, and I’m a manager at a multinational.
I want to tell you how an accident seventeen years ago could have destroyed me, and instead became the start of a new kind of awareness.
Life before the dental trauma
At 23 I had everything ahead of me: I was about to graduate in economics, I played volleyball semi-professionally three times a week, I had just started a serious relationship. My teeth were perfect, white and aligned — I’d never even had a cavity.

When I brought my hands to my face and looked at my fingers, there were fragments of teeth.
Losing the central incisors: the psychological shock
At the emergency room they dressed the wounds on my face, but for the teeth they told me: “You need to see a dentist urgently.”
I had completely lost two central incisors, and a lateral one was badly fractured. In a few seconds, my perfect smile was gone.
When I looked at myself in the mirror at the hospital, I didn’t recognize myself.
That kid with a huge gap where his front teeth used to be couldn’t be me. I’m only 23, I thought, I’m far too young to be toothless.
The emotional collapse after losing my teeth
The days that followed were the worst of my life up to that point.
I couldn’t speak without whistling, I couldn’t eat anything solid, but above all I didn’t want anyone to see me.
I stopped going to university for a few weeks; I told my volleyball team I couldn’t play anymore.
My girlfriend tried to stay close, but I kept shutting myself away. At 23, just as I was about to step into the working world, I found myself with a problem that felt insurmountable.

What the literature says: Luca’s case isn’t luck
Seventeen years on, it’s tempting to call it a miracle. It isn’t. It’s what the evidence predicts when the selection criteria are followed to the letter — and in Luca’s case they all were: twenty-three years old, excellent health, intact bone, teeth with no infection.
The systematic review by Wittneben and colleagues (Clinical Oral Implants Research, 2023), covering 1,961 implants, reports survival rates of 99.2% at one year and 95.8% at five years for implants placed and loaded immediately in the maxillary esthetic zone. Numbers that, given adequate primary stability, hold their own against any delayed protocol.
What makes the difference, though, is selection — not speed. Hamilton and colleagues (Clinical Oral Implants Research, 2023), as part of the ITI consensus, showed that the high survival of the “Type 1A” protocol — immediate placement plus immediate loading — is achieved only when patient and site are chosen by strict criteria: an intact buccal wall and the absence of endodontic infection shift the prognosis in your favour to a statistically significant degree. Luca fit that profile exactly. Not every patient with a tooth to replace is a candidate, and pretending otherwise would be dishonest.
Then there’s a detail patients rarely know. The fixed temporary fitted on the same day doesn’t just let you smile sooner: it protects the architecture of the tissues. The meta-analysis by Qin and colleagues (International Journal of Oral & Maxillofacial Implants, 2023) found that immediate loading preserves the midfacial mucosal level by roughly half a millimetre more than delayed loading. On central incisors, half a millimetre is the distance between a natural result and one that gets noticed. And when the site isn’t perfect? Even then the road stays open: Chen and colleagues (The Journal of Prosthetic Dentistry, 2024) documented a 98.1% survival rate for immediate implants placed even into sockets with bone defects in the esthetic zone.
Seventeen years later, the longevity of those implants surprises no one who knows the data. It only surprises those who were convinced the sole alternative was waiting months with a removable denture.
